In order to claim the part of the pension insurance that the intern has paid during the internship in Japan (from the time you pay social insurance),you need to complete the 4 types of documents listed below within 2 years from the date of your internship departure date from Japan:

You must fill in all the information in capital letters from items 1 to 5 of the application form, (if you do not complete the information, the application may be returned) . In Section 5 (Information of the allowance manual) section “Basic benefit number” : you need to look in the National Social Insurance Book to fill it out. This book must be brought back to the Philippines.

  • NATIONAL SOCIAL INSURANCE BOOK (年金(ねんきん)手帳(てちょう)) (copy).

PASSPORT PHOTOCOPY : Copy all pages with: name, nationality, signature, and pages stamped by Customs (Japan, Philippines) with the date of your entry and exit..

PHOTOCOPY OF BANK BOOK or CERTIFICATE OF THE BANK WITH INSPECTION: Name of the Bank, branch name, branch address, account number and full name of the account holder. Ngoài ra vui lòng lấy chứng nhận của Ngân hàng vào cột “Con dấu chứng nhận tài khoản của Ngân hàng” trong mục 4 của đơn đăng ký.

Things need to notice

  1. 1. Because you must send the original National Social Insurance book, when you go to the post office, please send it in the form of Secured Mail (ems).
  2. 2. Before putting in the envelope, please copy: APPLICATIONFOR FULL SUPPLEMENTAL Wage Allowance (completely filled out), and Social Security Number (Page with your name, age, and insurance number) (for for later comparison).
  3. 3. If the registrant dies without receiving the lump sum pension, the spouse, children, parents, grandchildren, grandparents, siblings together earn money to support the family at the time of the registrant’s death. on behalf of the recipient of the subsidy. (Applicable only in case the registrant himself filed the application before his death).
  4. 4. The pension insurance package from the national military allowance is not subject to income tax at source. Social welfare insurance is taxed at 20% at source upon payment. This tax can be refunded at the tax office. To claim this part of the tax before returning to your home country, you must submit the “Tax Manager Notice(納税(のうぜい)管理人(かんりにん)の届出書(とどけでしょ)) to the tax office of management area to appoint tax administrators. 5. The paper “Notice of decision on payment of lump sum pension benefit” (脱退(だったい)一時(いちじ)金(きん)支給(しきゅう)決定(けってい)通知書(つうちしょ))will be sent to the address in the Philipppines. (the address where you have filled out the application) at the same time as transferring the lump sum pension to your account. Then send the original of this notice to the tax administrator. The tax administrator will act on behalf of the registrant to process the tax refund.
